[PATCH] Automatic creation of identity model tables for SQLAlchemy

For SQLObject, the visit/identity tables are created automatically, but for SQLAlchemy this has so far not been implemented. This is also the reason why the controller test for a newly created project with SQLAlchemy does not work.
The patch adds automatic creation of the necessary tables with SQLAlchemy (if they do not exist already), similarly to how it is done with SQLObject. It also gets rid of ActiveMapper? in the visit provider since this has been removed in the standard model.py as well. The patch can be applied to both 1.0 and the trunk.
