[Patch] Adding FastCGI instructions

The attached patch adds information about running TurboGears as FastCGI. The instructions are very similar to  http://wiki.pylonshq.com/display/pylonscookbook/Production+Deployment+Using+Apache,+FastCGI+and+mod_rewrite , but for someone who is not necessarily familiar with the internals of TurboGears, it's not immediately obvious that something for Pylons would work for TurboGears. Also, there are some slight modifications to the boilerplate dispatch.fcgi script to make it work well with TurboGears

The path fixing and close connection middleware doesn't work. I've written an alternate middleware that fixes the former, but I think it should really be included in the default TurboGears middleware if some option, say, FastCGI, is specified.
